General Improvements and Fixes

Improved the performance of Issue creation for enterprise scale Users with hundreds of Fields.

Fixed an issue seen on Responsibilities Maps whereby tables with large Responsibility descriptions were sometimes seeing truncated data in the PDF output.

Fixed a rare issue seen when transitioning to a new Issue stage after Issue creation, whereby additional transitioning was possible over and above that specified before a refresh.
